It is common that people diagnosed with HIV/AIDS face many barriers to continued participation in medical care. Homelessness, substance use, mental health, poverty, unemployment, lack of health insurance, lack of knowledge about chronic illnesses and domestic violence are just a few of these barriers.

MFAP aims to help identify and address barriers to care to help ensure participants stay healthy.

MFAP provides HIV testing, counseling, referrals, and early intervention services including medical case management, treatment adherence, mental health/nutritional counseling.
